Jennifer Srock, Outreach Coordinator at Community Partners, has been with Orleans Community Health for nearly seven years.Jennifer works primarily in overseeing Community Partner’s Cancer Services Program, a grant-funded program that has been in place for over 20 years.
The Cancer Services Program provides delivery of comprehensive breast, cervical and colorectal cancer screening and diagnostic services to eligible uninsured and underinsured individuals in our community. Jennifer develops relationships with regional providers, for example, hospitals, clinics, health care providers, and community-based organizations to collaboratively conduct outreach to priority populations for screening, diagnostic, and case management services. Jennifer also performs quality assurance monitoring, public education, and data management, and other activities outlined in the Cancer Services Program grant contract.
Jennifer is a DQE (Designated Qualified Entities) DQE’s are individuals authorized to complete applications for enrollment in the Medicaid Cancer Treatment Program. Jennifer assists individuals diagnosed with breast, cervical, colorectal or prostate cancer in obtaining prompt, comprehensive treatment through the New York State Medicaid Cancer Treatment Program (MCTP), if eligible. Eligible individuals may receive full Medicaid coverage for the duration of their cancer treatment.
Jennifer helps to ensure that the many Cancer Services Program goals and objectives are met by monitoring, reviewing and revising activities according to monthly performance measure reports, budget monitoring tools and other performance indicators.
One of Jennifer’s favorite parts of her job is helping to coordinate For Women Only. For Women Only is a fundraiser event used to provide services for men and women in our community that the Cancer Services Program does not cover. For Women Only provides education on breast cancer, highlights cancer survivors, and is a fun night out for women with a Chinese Auction and vendor booths.
Jennifer has worked on several other grants and initiatives while working at Orleans Community Health. Jennifer has an Associate’s Degree in Sales and Marketing from Bryant & Stratton, has taken courses at Niagara University and is currently enrolled at Empire State College. Jennifer worked in the banking industry and for several years doing sales and marketing activities for a local builder before coming to Orleans Community Health.
Jennifer is from Middleport, NY, where she lives with her husband Fred and her three teenage boys. She spends most of her free time enjoying and watching her children’s sporting and go-kart racing activities. She is also involved in both Cub Scout Pack 23 and Boy Scout Troop 23 as a committee member.
